TERMS AND CONDITIONS

1. Users must be at least 18 years old.
2. Users are not allowed to transmit materials that are slanderous, obscure, threatening, defamatory and obscenes
3. Users must not provide fraudulent information such as creating false user information or registering for another person.
4. Users must not hold Citi Wellness Center, its directors, agents, partners, employees, affiliates and associates liable for damages for the information, services and products provided by the website. If you are not amenable to the services provided by the website, please feel free to inform management and immediately stop using the website.
5. Users assume all risks, hazards and responsibilities encountered when viewing the content of the website. Users assume full responsibility for one's actions.

CLIENT RESPONSIBILITY

1. Client Shall Seek Independent Advice.
2. Client Shall Make Informed Choices Regarding Services.
3. Client Shall Provide Complete and Accurate Information to World Citi Medical Center and All Providers
4. Client Shall Assume All Risks.
5. Client Shall Indemnify World Citi Medical Center.

This website has no liability, commitment, warranty, guarantee to any user, reader, client of the server and any such agreement, commitment or understanding shall be the sole responsibility of the server.

Coordination Services. In booking direct services and assisting in the coordination of such services between user and third parties, World Citi Medical Center and Citi Wellness Center are acting solely as independent contractors and not as an agent, employee, or joint venture partner of other third party service providers, and if applicable, third party service provider affiliates.

Independent Contractors. The service providers (parties) are all independent contractors. Nothing in this terms and conditions shall be construed to create any venture, partnership, or entity that is inconsistent with the Parties� status as independent contractors. World Citi Medical Center and Citi Wellness Center shall provide Coordination Services.

Neither Party shall take any action or make any representation that would suggest that any relationship exists between the Parties other than that of independent contractors.

Neither Party shall have any right or authority to assume or create any obligations on behalf of the other Party, express or implied, nor shall either Party represent to any person that the Party has such authority or that the Party serves the other Party in any capacity other than as an independent contractor.

Information on Direct Provider(s). World Citi Medical Center and Citi Wellnes Center do not recommend any preferred or specific physician, hospital, or other health care provider for Client�s desired health care treatment, including Direct Care Services. World Citi Medical Center�s responsibilities are limited to facilitating contact with, and providing information about, one or more physicians, hospitals or other health care providers within the Philippines, in Client�s home country and/or abroad including their credentials, to assist Client in making informed decisions.

No Guarantee Regarding Provider Services. Except as otherwise provided herein and to the fullest extent permissible by law, World Citi Medical Center and Citi Wellness Center through the provision of Coordination Services shall not be deemed to represent, warrant, or guarantee the cost, appropriateness, efficacy, quality, result, or outcome of any Provider Services, including the result of any Medical Services.

Warranty Disclaimer. Except as otherwise provided herein and to the fullest extent permissible by law, World Citi Medical Center and Citi Wellness Center assume no responsibility nor does it grant any warranties, express or implied, relating to the operation, safety, condition or service of any physician, hospital, other health care provider, aircraft, motor vehicle, boat, other conveyance, inn, lodge, or hotel and/or products associated with such entities or services that are used wholly or in part by, for or on behalf of Client or Client�s accompanying person(s). World Citi Medical Center and Citi Wellness Center are not liable for (a) the acts, errors, omissions, representations, warranties, breaches, negligence or professional malpractice of any provider or any personal injuries, death, property damage or other damages or expenses resulting from the acts or omissions of the providers or any other causes outside of World Citi Medical Center and Citi Wellness Center�s control, or (b) any liability, claims or damages arising out of or related to Client�s health, medical history or condition, or the health care treatment of Client, from any cause whatsoever.

Limited Liability. World Citi Medical Center and Citi Wellness Center shall not be liable for any injuries, damages, or losses caused to Client in connection with, or by the employees or contractors of providers, terrorist activities, social or labor unrest, mechanical or construction failures, or difficulties, diseases, local laws, climactic conditions, criminal acts or abnormal conditions or developments, or any other actions, omissions, or conditions outside the control of World Citi Medical Center and Citi Wellness Center.

Limited Damages. World Citi Medical Center and Citi Wellness Center shall not be liable to client or any accompanying person (or to any person claiming rights derived thereunder) in contract, tort or otherwise for any damages whatsoever, including, without limitation, actual, economic, incidental, consequential, special, punitive, or exemplary damages of any kind including, without limitation, emotional distress or loss of consortium.

Confidential Information. "Confidential Information" means all non-public information furnished by a Party (the "Disclosing Party") to the other Party (the "Receiving Party") in performance of this Agreement. Except as required by law, Receiving Party shall not disclose to any third party that Receiving Party received the Confidential Information from Disclosing Party. Except as required by law, Receiving Party shall not disclose, and shall not permit to be disclosed, Confidential Information to any third party, including disclosure of any data or document (in any form) that contains or reflects Confidential Information and including any analysis or derivative of the Confidential Information.

Governing Law. This Agreement shall be governed by and construed in accordance with the laws of the Republic of the Philippines without giving effect to the conflicts of laws principles.
